Welcome to Kiwi Pediatrics! We look forward to providing health care to your child/children for many years to come. At your first appointment we will provide you with information about our practice, office policies, HIPAA (Health Insurance Portability and Accountability Act) and some disclosure forms.
Please complete this downloadable registration form before your first visit with us. Because this is your child's first visit to our practice, please arrive fifteen minutes before your appointment time so we can verify your insurance and complete your registration. It is helpful if you are able to bring in your child’s/children’s immunization card and any medical records you may have.
